COVID-19 Daily Health Check
As part of Urban Rec’s Return To Play (RTP) Protocol, we require that all players that are attending a game adhere to the pre arrival health check and that team captains acknowledge that they have confirmed with their team the answers to the questions posed below.
Team captains will be sent the following questions, via email, that they are required to ask all players that will be participating that night. Team captains will be required to respond to this email prior to them being able to participate on any given night of play. This health check will be in effect until the pandemic is deemed over by the relevant health agencies.
Please confirm that all players on your team who will be attending tonight’s game answer “NO” to the following 4 questions:
- Have you experienced any symptoms of COVID-19 (which may include but not be limited to fever, chills, cough, shortness of breath, sore throat, painful swallowing, loss of sense of smell) over the last 5 days?
- Have you travelled to any country outside Canada within the last 14 days?
- Have you provided care, or had contact with a person confirmed to have COVID-19, in the last 5 days?
- Do you have any reason to believe that the answers to the questions above are inaccurate or untruthful, understanding that their accuracy and truthfulness are the basis upon which we can all play safely, and further understanding the indemnity provisions agreed to by all Urban Rec participants during the registration process.
Anyone who answers “YES” to any of these questions is unable to attend tonight’s game.
